No Refund Policy
Medical injectable services administered are not refundable. I strive my best to ensure my clients are educated in regards to outcomes. However, every client will utilize the Botox or Dysport, or Fillers differently. Everyone is unique, therefore outcomes can not be guaranteed.
The risk and complications of all procedure will be explained prior to treatment.
Allergic reaction will be treated and any event of an allergic reaction or adverse reaction is a risk with every client.
Experiencing such a event will not constitute a refund of the completed treatment. Client agrees to the injection treatment knowing the risk and has realistic expectations of the results and risk.
All injections have a risk of causing trauma and bruising along with swelling, redness and tenderness. These reactions are expected and normal with aesthetic injectables.
Aesthetic medications are administered include on-label and off-label treatment areas based on the client’s desire and need as ordered.
Online Forms
You will be sent online forms through a health care appointment booking, and form completing platform.
Consent Forms
You will be sent forms of consent for every treatment you would like to have done, prior to your appointment. This is for you to review and have questions ready when we meet in person.
If you are seeing me in a clinic other than Vaughan, you will need to have a teleconference with my medical director. This is mandatory for all new clients, and old clients performing new treatments.
Client confidentiality
If you would like to share your medical records with another provider, a written consent must be sent to me in advance.
I do not treat minors under the age of 18.
Children under 12 cannot accompany clients into the treatment room or in the medical clinic.
If you have any additional questions or requests, please feel free to inquire before your appointment.
Once we schedule an appointment, please be on time. If you are 15 minutes late, your appointment may be cancelled as there may be another appointment after you.
$50 deposit require to book an appointment.
The $50 will be applied to your treatment. If you chose to not have a treatment, you will be refunded.
Payment is due at the time of service. I accept debit, all major credit cards.
Unfortunately, I do not accept EMT's, checks or payment plans.
You may finance your treatment with our partner @dofinance
If you are a new client, we try our best to appropriately dose you.
A follow-up at 2 weeks is mandatory if you would like an adjustment. Anything past that time frame must wait until the wrinkle relaxer has worn off (3-4 months)
